Required Documents: Phase-II CAB Designation to IDA Singapore

The following list of documents must be submitted for U.S. CAB nomination under Phase II of the APEC TEL MRA for IMDA Singapore requirements:

  1. A cover letter or e-mail that includes the following:

    Request to NIST for CAB nomination to IMDA Singapore under Phase-II of the APEC Tel MRA
    Primary contact person's name, mailing address, phone number including extension (if any), email
    Alternate contact person's name, mailing address, phone number including extension (if any), email
    Desired IMDA Scope for Phase-II

  2. A copy of the CB's Scope and Certificate of Accreditation to ISO/IEC 17065 referencing the relevant IMDA technical specifications.
  3. A copy of the CB's associated testing laboratory Scope and Certificate of Accreditation to ISO/IEC 17025 referencing the relevant IMDA technical specifications.
  4. If the CB is accepting test data from other accredited testing laboratories, include:
    A copy of the contractual arrangements made with each testing laboratory
    The Scope and Certificate of Accreditation for each testing laboratory listing the relevant IMDA technical specifications
