On-Site Assessment Forms

This page provides links to the forms that are used by a NVLAP assessor to record the results of a laboratory's on-site assessment. These forms constitute the "On-Site Assessment Report."

Assessors use checklists to ensure that each laboratory receives an assessment comparable to that received by others. All NVLAP programs use the NVLAP General Criteria Checklist, which contains the requirements published in ISO/IEC 17025 and NIST Handbook 150 Annexes A, B, and E. The NIST Handbook 150-xx series checklists (Program-Specific Checklists) address requirements that are specific to a particular field of accreditation.

The Test Method Review Summary, used in conjunction with the checklists in most testing programs, is a tool for capturing information about the test methods assessed at a laboratory and the extent of the assessment. For each test method, the assessor records the depth of the assessment and other pertinent information.

Unless marked otherwise, each form listed is the latest controlled version which is also available in the NIWS Assessor Portal.
